REFERRALS

We require a referral letter in order to book you an appointment with Mr Karpik. This letter can come from your general practitioner, physiotherapist, sports doctor or other medical practitioner.

Referral Expectations.
Please ensure that the following information is either present on your referral letter or bring copies of records to your consultation:

• Any letters or reports from your doctor or other hospital.
• Any recent X-Rays, Ultra-sounds, CT or MRI films and reports.
• A list of all medicines you are taking including herbal and natural remedies.
• If claiming under ACC, we require your ACC number and the date of injury.

Booking an Appointment.
Once we receive your referral, we will contact you to schedule an appointment.

If you have had any relevant X-ray, Ultrasound, MRI or CT scans, you will need to provide the name of the radiology group where you had these performed.

If you have Southern Cross Health Insurance we require your policy number in order to obtain prior approval for your consultation. Depending on your policy there may be part payment for your treatment.

If this appointment is related to an ACC Claim, we require the ACC Claim Number and Date of Injury.
Prior to your appointment, please ensure that your claim has been accepted by ACC.
If we do not have this information, or your claim is declined, you will be invoiced for services received.
